PrimeCare accepts most major insurance coverage, including Medicaid.  Because of the Affordable Care Act, the insurance marketplace is undergoing significant transformations.  The plans listed below are some of the plans we currently accept, but due to fluctuations in the insurance marketplace, this will likely change over time.   Be sure to bring your insurance card with you to your next visit, so we can verify acceptance.

PrimeCare Outreach and Enrollment Specialists can help you find the health care insurance option that best suits your needs. They will review available plans with you, including those offered through the Affordable Care Act (ACA) and Medicaid. Once you decide which one you want, they will help you with the enrollment process. You don’t have to navigate the system alone. Financial Assistance

For eligible patients, PrimeCare’s services are offered on a sliding fee discount program. No patient will be denied health care services due to their inability to pay for such services. Any fees or payments required by PrimeCare will be reduced or waived to make this possible. Eligibility for discounts is based solely on income and family size – not whether or not you have insurance. Here’s the scale for 2017: To see where your household income falls on this scale, click here. In order to receive these discounts, you will need to establish your eligibility by filling out an application. You can view this application here. At the bottom of the first page are the documents you will need to bring with you to complete your application. Because the document must be signed, you will have to fill it out when you come for a visit. Eligibility is determined on an annual basis, and you will need to reapply every year.
